枫下家园 / 家庭与子女 / 请教关于新生BB的各种文件填表的几个问题。1。在Request for birth Certificate 的第一栏中，Applicat's name是写BB的名字，还是母亲的名字？ 因为表上写：In the context of this form, the word "Applicant" refers to the person completing this equest. This may or may not be the "Person named on t he Birth certificate". 不知道是否应该写妈妈的名字。
2。还有一个是可以申请加急“request for birth certificate”的表，问为什么申请birth certificate,是否可以写：new born baby？
