With formal birth certificate, the passport can be extended for two years more. In my opinion, you can extend the passport at local Canadian Embassy under this situation.

通常small = short form用来办SIN卡，large = long form用来办护照，牛奶金，都需要办千万别只办一个，以免以后麻烦

I think large form can be used as short form, but short form can not be used as large form. Anyway, SIN card will only be usefull when the baby growns up.
