1) "citizen" 2) leave it blank 3) don't know your situation. 4) I think it's useless for yousince it's not you to apply the visa and it's not related to your financials, which should be kept in your mind as the most important to the visa officer.
As told by an assistant to visa officer for filling forms:
Actually some information is not very critical . Finacials are the number one and the tendency to immigration is always an excuse. So don't tangle in trival details too much while loosing big picture.

I can only tell the general rules. The results of visa application are to volatile in time, locations and individuals. Anyway, if you don't provide your own information to officer, that's ridiculous to him too.
